Making
Appointments
Please be sure to arrive at least 15 minutes prior to your
scheduled appointment and bring the following items with
you:

I.D. Card for registration (driver’s license, birth
certificate, social security card, or another form of
official identification)

Immunization Records
-
Billing information (Med-Cal / Medicare card or private
insurance card)
-
All medication containers
If
you are without insurance please ask one of our front office
staff if you qualify for our Sliding Fee Scale Discount Program.
Walk-In
Appointments
We recommend that you call or come in to schedule an appointment
however we will make our best efforts to accommodate same-day
